Otoplasty is an operation to permanently correct, reposition or reshape protruding or malformed ears. It can also improve the aesthetic appearance (projection and/or shape) of the ears, offering a natural result.

The malformation is congenital and has been present since childhood. It is most often bilateral, but sometimes affects only one ear. The operation can be performed under local anaesthetic, but may sometimes require a more extensive anaesthetic.

Procedure

Before the operation, a pre-operative consultation is compulsory. If the presence of an anaesthetist is required, a consultation with him or her will also be arranged. Otoplasty is usually performed in the operating theatre on an outpatient basis, with discharge authorised on the same day, but in some cases a short stay in hospital may be necessary.

 

During the operation, the cosmetic surgeon will choose the appropriate technique depending on the defects to be corrected and the chosen surgical strategy. The operation, which lasts between 1 and 2 hours, involves making an incision behind the ear or at the back of the ear. The skin is then removed as required, and the cartilage remodelled.

Results

It takes one to two months to appreciate the final result. This is the time needed for the tissues to soften and for all the oedema to subside, leaving the contours of the ear clearly visible. After this time, only the scars will still be slightly pink and indurated before fading, after about 12 months.

 

The aim of all surgery is to improve, not to achieve perfection. As long as the patient's wishes remain realistic, the result should give great satisfaction. In the case of otoplasty, it is important to be aware that a touch-up operation may be necessary if a stitch on the cartilage breaks, and that hypertrophic scars are possible, particularly in young children.

Pre-operative precautions

  • A pre-operative blood test may be carried out to detect an increased risk of bleeding.
  • Four weeks before the operation, it is essential to stop smoking.
  • 14 days before the operation, you should stop taking aspirin and non-steroidal anti-inflammatory drugs (NSAIDs, ibuprofen, diclofenac), as well as turmeric, omega-3 and vitamin C.
  • The day before the operation, wash your hair and face thoroughly.
  • On the morning of the operation: clean your ears thoroughly with soap and water (in the ear canal as well as in the front and back). Remove all clips and piercings.

Post-operative care

Typically, absorbable sutures are used, which fall out within 2 to 3 weeks; if not, they will have to be removed around the tenth day. The after-effects of otoplasty are not very painful and can be treated with painkillers if necessary. Swelling and bruising of varying degrees are temporary.

 

Once the first bandage has been removed, it will be replaced by a lighter bandage for a few days, then by a protective bandage. This should be worn day and night for a fortnight, then only at night for a further four to six weeks. During this period, physical or sporting activities involving the risk of contact should be avoided, as should exposure to extreme cold.

 

Earrings and headphones should not be worn for a fortnight after treatment, and care should be taken when using the telephone for four weeks after the operation, to avoid the risk of infection. To avoid disrupting the healing process, you should sleep on your back for about 4 weeks, preferably using an inflatable travel pillow.
